The hidden cost of scaling empathy
Creating trauma-informed content isn’t just intellectually demanding, it’s emotionally devastating. The consistency that AI provides makes broader testing possible without increasing human exposure to traumatic material. Traditional user testing with trauma-informed content presents ethical challenges. We don’t want to expose vulnerable users to potentially triggering content. AI sentiment analysis can evaluate content against trauma-informed principles before any human exposure, allowing for broader testing and refinement cycles.

Someone experiencing relationship difficulties may benefit from more empathetic language and additional support resources. The system can recognize patterns of engagement that suggest microtrauma, including shorter session times, rapid clicking, or abandonment at specific points, and adjust accordingly.
This level of personalization can significantly improve user engagement. In my own work, trauma-informed approaches have reduced user errors by up to 50%, and I’ve seen strong engagement gains as a result. In this approach, human designers guide the AI by defining trauma-informed principles, while the AI assists in generating content that adheres to these principles. The critical insight is that each stage builds on the previous one, creating a protective buffer that grows stronger as the process develops.
Measuring empathy at scale
To track whether this AI-supported approach is effective, we need clear metrics that focus on empathy and scalability. Coverage metrics indicate the percentage of content that has been evaluated against trauma-informed principles, while consistency scores assess the uniformity with which these principles are applied across content.
For measuring secondary trauma among content designers, implement simple daily or weekly self-reporting surveys that track stress levels and emotional strain. These questions need to be specific to trauma-informed content work:
- How often did you feel emotionally drained after reviewing content this week?
- Did you experience intrusive thoughts related to user scenarios after work?
User abandonment rates at potentially triggering points indicate where content might be failing users. Personalization reach indicates the number of users receiving tailored, trauma-informed experiences.
When measuring success, account for how microtrauma impacts user engagement. Metrics should capture not only user abandonment at critical points but also assess how microstressors affect users’ attention span, interaction rate, and emotional state across the entire content journey.
Establish real-time feedback loops that can trigger immediate adjustments. If a particular type of trauma-related query sees a sudden surge, AI models can be rapidly updated with user feedback, ensuring content remains aligned with users’ immediate emotional needs.
Addressing the most important concerns
The idea of using AI to scale empathy raises legitimate concerns, but the most critical comes from Janice Hannaway, a trauma-informed user researcher who worked with me at the UK Cabinet Office: “I feel there is still a risk of the user experiencing a traumatic response, for example dissociation, and I feel that only a human could pick up on this and ensure the safety of the user.”
This highlights AI’s most significant limitation. AI cannot detect real-time trauma responses, like dissociation or emotional shutdown, that may occur during a user’s experience. Real-time human sensitivity remains paramount in these moments when only a human can assess the situation and intervene effectively.
AI is a preparatory tool to ease the designer’s workload, but it cannot replicate the subtle, empathetic responses necessary in delicate moments. In cases of real-time trauma responses, AI can signal content designers to intervene, alerting them to patterns that require human empathy and expertise.
Will AI-scaled empathy feel authentic to users? The authenticity comes from human designers who define the principles and review AI-generated content. By using AI for consistency checking and trigger identification, human designers can focus their empathy on the most critical aspects of content creation.
Transparency in AI systems is vital. Content teams must communicate clearly with users about data use, especially in sensitive situations. Clear privacy policies and informed consent ensure that users understand their rights and the role of AI in their experiences.
How to get started
Step 1: Document your principles
To begin scaling empathy through AI, document your trauma-informed principles in detail. Create clear guidelines that define what trauma-informed content looks like in your context.
Step 2: Test with the right users
Test these principles with users who have lived experience of trauma, including both those who experienced traumatic events in the past and those currently dealing with difficult situations. This distinction matters because someone who experienced domestic violence five years ago will engage with content very differently than someone currently in an abusive relationship.
Step 3: Start with low-risk scenarios
Begin with non-user-facing applications that protect your team while building confidence. Test how AI responds when you ask it to identify potentially triggering language in benefits application content, then compare its flagging against your own trauma-informed principles.

Ensure your team has access to professional support, implement regular wellness check-ins, and foster openness around secondary trauma. Start small, measure carefully, and scale gradually as you build confidence in your approach.
The empathetic path forward
